Reviewing an Existing Estate Plan: How to Use the Estate Plan Review Questionnaire

If you already have an estate plan, such as a will, a living trust, health care documents or powers of attorney, and you want it reviewed, the Estate Plan Review questionnaire is the form to fill out. This guide explains what it asks.

Which questionnaire do you need?

Your information

Enter your first and last name, add any additional people if you are a married couple, the date of your consultation (which you arranged with my office in advance by phone), how you learned about us, and your best address, phone number and email. The email is how we reach you, and a copy of your completed questionnaire is sent to it.

Which documents you have

An estate plan is a collection of legal tools, including a living trust, a last will and testament, powers of attorney and health care documents. Tell us which of these you have. If you are not sure what they are, say so. If there are other documents to review, list those too.

The goal of the meeting

The form asks what you hope to get out of the review. You may want to make changes, it may have been too long since you looked at the documents, you may want to check on the plan, or you may not know what it says and need an explanation. If you are not sure, say that. This gives me a heads-up so I can focus the meeting. List any changes you want to make and any additional concerns, and anything else you think we should know.

Sending your documents

Upload your documents with the upload button or by dragging them into the field. If the files are too large for the form, free services such as filemail.com or wetransfer.com can send us a large amount of documents. You can also drop the documents off at the office or email them to us.

After you submit

You will see a confirmation page you can print and you will receive a copy of your responses at the email you provided.
