How to Get Started!

1. First, call our office to set up your appointment.  Please call (661) 414-7100 or (800) 799-7449.

2. Download the correct form (see right) and return it to our office a few days before your appointment.  You can mail it, or you can fax it to (866) 481-4265.  By providing the information in advance, our initial appointment will be more productive.  

You will need to open the form with Adobe Acrobat software to do so.  Most computers have this software already installed. Please mail the completed form to us in advance of our meeting whenever possible.  That way, our meeting can be more productive.
