Get Started with Your Estate Plan!

1) First, call our office to set up your appointment.  Please call (661) 414-7100 or (800) 799-7449.

2) Download the correct Estate Planning Questionnaire (see below) and return it to our office before your appointment.  You can also fax it to (866) 481-4265. These forms are "fillable," so if you wish, you can fill them out on your computer, print them, then mail them.  Otherwise, just print them out and fill them out by hand.  Then mail the form to us in advance of our meeting.

3) Or, instead of downloading the form, you can also fill out the quick secure online form* by clicking here.  (If you are asked to display "nonsecure" items, click "No")

*The online form is quick but not as complete as the downloadable forms.  If you are short on time, the online form is a quicker way to get started. During our initial meeting, we will discuss everything together.
